Getting a Medical Marijuana Card

Before Your Appointment

  1. Consider if you are a good fit for a MM card. It may be helpful to review our Appropriate Candidates for a MM Card page before proceeding. 

  2. If you believe that you are an appropriate candidate, please make sure that you have a documented diagnosis from your medical or mental health provider. 

  3. Complete your state registration at the Maryland Medical Cannabis Commission website and pay your card registration fee ($50). This is a state fee. It is not a fee set by iHEAL. 

How To Make An Initial Evaluation Appointment

  • You can call to schedule an appointment (410) 864-0211. 

  • You can also use the Book an Appointment link. Complete the form and then we will call you to confirm your appointment time and take payment over the phone.

 

***Please note that you will need your insurance card information when you make your appointment. If you are paying out-of-pocket, you must pay the initial appointment fee ($200) before securing an appointment. This fee includes all of the services listed on our MM Card Program Specifics page. 

What You Need for Your Initial Evaluation Appointment

  1. You will need proof of identification (copies are not accepted)

    • Valid Maryland driver's license​

    • Official passport

    • Maryland identification card

  2. Social Security Number

  3. Registration code (16-digits) received from the Maryland Medical Cannabis Commission (if you have it).

  4. Medical documentation of your diagnosis​***

    • Letter from your medical or mental health provider​ OR

    • List of prescriptions (from medical or mental health provider on official letterhead or from your pharmacy portal)​​

  5. Dress in a solid, light-colored or white shirt.​​

***If you are a current iHEAL patient and actively involved in mental health treatment, you don't need to provide medical documentation as we already have access to your mental health diagnoses.

Activating Your MM Card

  • If you receive your 16-digit MM registration code BEFORE your initial medical evaluation...

    • bring it with you to your appointment. If you are deemed appropriate for a MM card, we will activate your registration code at the end of the appointment.

  • If you receive your 16-digit MM registration code AFTER your initial medical evaluation...

    • you can complete the MM Card Registration Form. If you are deemed appropriate for a MM card, we will activate your registration code within 48 business hours. You should receive an email confirmation when your card has been activated.
